If they're inherently incompatible, how have they been working together so long and why aren't they failing?

They've only recently started expanding and the expansion hasn't reached critical mass yet. Not even close. Lots of cultural things work that way. They start out small and seemingly innocuous. This role problem I'm describing, following its current path, won't likely reach critical mass in my lifetime. But that doesn't make distorting roles a good idea.

You have not addressed the value added from a hospital's affiliation with the Church. I see no value added, health care wise. If there's no value added, then it's just a power thing.
