Yes. In hindsight I wish I would have folded up the questionnaire and put it in my pocket. I am sure no one would have noticed while we were there.

In my defence, I was somewhat preoccupied with what the specialists were going to tell me than a silly and intrusive questionnaire.

My wife and daughter were with me and we had some belly laughs with it which was a pleasant distraction.
