The point that is lost in all this is what exactly health insurance (or any insurance) is there for -- to enable people to spread risk amongst a large pool.

Any time health insurance companies provide coverage for an item that doesn't reduce their costs elsewhere, it is going to increase health insurance costs to the pool.
