I am convinced that the best way to go is to join an HMO. I have a great one here with Kaiser. If I get in physical trouble, I will arrange to stay here because of the climate and the built in medical care. You will need more than just an MD. I see my MD once every couple of years. Deal with him by Email and phone, which you can do in an HMO situation. The spencialists are who you will want to see personally.
My wife developed an infection that went systemic about 6 years ago when we were on Maui. She was seen initially by docs at Kaiser and then admitted to Maui General. Not much fun for her to be in hospital while on vacation, but we were very impressed with the way Kaiser handled the situation. We paid very, very little out of pocket for her treatment (it was coordinated with our Group Health policy from Washington).
