Assume you are retiring. Assume you don't have to live where your kids are. Pick a warm climate with a good HMO.

The warm climate definitively adds years to your life.

So will a good hospital referral system. The HMO has the internal checks to keep the incompetents off their staff. There is no way you can tell.

If you HAVE to live near your kids, then check the HMO's by you.

But stay away from the independent Doctor.
