You are being technical. I was using the label broadly, merely to signify the central payer paying less than the provider asks.

Rationing is suboptimal

I don't think so. If the system is not feasible without it, then it's necessary. If it's necessary, it can't be suboptimal. It may be suboptimal to operate a centrally controlled system but that's beside the point. If you're going to establish a workable centralized system, then you cannot allow ad libitum usage. If you don't allow ad libitum usage, then you have rationing, again, using the term broadly.
