So, if you are an executive and one branch of your organization is bubbling over with potential violence and the person in charge of that branch doesn't inform you that there is a threat to the overall organization, hasn't the executive failed by not controlling his subordinates in such a way that he is not blindsided by a complete clusterf*ck?

The buck stops at the top, even if he didn't know about it. Because not knowing about it is an indication of a different kind of failure.

Hillary may be falling on her sword due to her loyalty the the party, but that doesn't change Obama's culpability in the situation.
