I guess some clarification of my post is in order. What I meant by " they did not know of this possibility" was that they did not know that an appropriate statistical technique could accomplish the adjustment.
What I meant by the data lacking quality is simply incomplete data which can readily happen with patient diary data. I too suspect that possibility 2 is much more likely.
