He is referring to the bystander effect I think. The treated lesion was fixed, but something made it to the lung. Considering that the immune system may take a bit to get going for the bystander effect, there is not enough data to make that assumption that it really failed. Did the met go away after the end of the observation period? Did they go after the met before the bystander effect kicked in? What was the time between the treated lesion and the time where the met was observed. Not enough data to make any assumptions.
