Yet, we know that private insurers in the US can finance health care more cost-effectively than either Medicare or Medicaid.

You can't compare systems from country-to-country. You just can't do it. There are too many variables, too many parameters, and too many fundamental differences to have any sensible outcome.

What you CAN do is modify the US system a little at a time to see how changes affect outcomes. Unfortunately, the nitwits in DC didn't do it that way and now we have a mess.
