The state didn't inspect clinics because it was a holy cause. Too holy to inspect. Gosnell was only busted because of drug trafficing violations. It was as a result of that investigation all the other stuff came out.
During the drug trafficking investigation, they learned that “Gosnell routinely relied on unlicensed and untrained staff to treat patients, conduct medical tests, and administer medications without supervision. Even more alarmingly, Gosnell instructed unlicensed workers to sedate patients with dangerous drugs in his absence.”

Abortion clinics probably aren't inspected most places now. Pro-choice groups oppose any inspections of the clinics as being harassment. It's prolife groups pushing for the same type inspections other health care facilities see for abortion clinics. Example:
