I think that - even if somewhat indirectly - one of the central reasons for the unexpected increase in costs was the advent of computers. They played into the human passion to have things organized - and under "control". Most importantly, they gave politicians and government - and private - bureaucrats a feeling of empowerment. Computers led them to think that it may be possible to create - and efficiently manage - complex social systems.

The result is the current (very expensive) complexity. We are about to see this experiment reach new levels under ObamaCare.
