The basic flaw in the argument for a vaccine-like immune mediated effect is that everyone understand that the immune system failed (some how) to detect and aggressively eliminate the 'foreign' cancer cells. The vaccine based approach assumes that despite the underlying defect, you can pump the system up (by presenting novel antigens) and kick start the cel killing response... clinical success along this line has been very limited and is old news. More recently, interest has been drawn to re-tuning the immune system to achieve greater killing effectiveness; immune interaction modulators like PD-1. Similar success has been shown with the anti-TNF therapies (which re-balance th1 and th2 responses) for autoimmune disorders. The simple minded vaccine based approach will likely never yield a strong, generalized clinical story; as allovectin has so amply shown. The good news for PV-10 is that the drug is modestly cytotoxic, so a simple de-bulking event can be achieved by IL-injection; which is probably why the company is so desperate to inject as many visible tumors in PIII as possible; there is a rough correlation between overall tumor burden and survival that they can hope to play to, although a systemic therapy might prove to be as good.
