I am very sympathetic to the real needs, and obviously that is something the ACA is trying to address. All I was trying to point out was the original issue, namely that indirection in payment for services can cause problems. That is true for any insurance system, and will be a problem for ACA too. I don't see any means of avoiding it, other than keeping substantial skin in the game. Its no different than what we saw with housing recently.
