The article seems to me to create more confusion than the system itself, by calling these billed amounts "costs". No one pays that. Even the uninsured get lower rates (the article references one self-pay bill that was discounted 65%). Medicare and Medicaid are going to pay essentially no markup; other insurances will negotiate the entire bill way, way down.

Personally, I think $1,400 for a basic self-pay ER visit isn't an insane amount of money, in fact, it seems like quite a bargain to me. They're staffing a facility with one or more doctors, nurses, access to lab and imaging systems, 24/7. And they'll see you whether you have insurance, the money to pay, or even the cognitive ability to discuss payment arrangements. If necessary, they'll drop everything they're doing to attend to your situation.
