So, are you in favor of a tiered system, where Medicare/Medicaid patients get one level of service and privately insured get a higher level of service?

If we have to have government health care, perhaps we should just codify it. Medicare/Medicaids can see NPs and PAs, but not MDs, unless the NP or PA refers to a real doctor. MCR/MCD gets only generic drugs, while those with money can get brands that aren't off-patent.

MCR/MCD -- 1.5t imaging ONLY. If you're on private insurance, you can get 3t imaging if your insurance pays for it. Hip/knee joint replacements? Cheaper joints for you. Cover dental work -- extractions only for MCR/MCD, but others can get crowns.

That seems to be what you are advocating.
