Because everybody is required to be 'registered' and have insurance. At present, you don't even need a SS number to get ER service. To whom will a hospital send the ER bill? Will it be payed? Hospitals know how to do that now and they get payed by a state or maybe even federal fund.

After all, a reason for the ACA is to provide care ( remember the name: Affordable Care Act ) to folks that previously had access (guaranteed access) to ER care only. Now they will have 'insurance'. But, of course, insurance only has to do with payment of money, not medical care.

So if the hospital can't get you to tell them what insurance you have or if you don't have insurance, will the hospital get payed? They certainly can't just tell you to get lost. By whom will they be payed? If they can't get these payments, will they then need to reduce ER staff and so not be able to meet real emergencies?

I think the present system has many benefits that should not be ignored.
