..nor should they be viewed as such

You either define a population for which the costs and benefits are shared by all members or you do not

If the population of the United States is to covered by a system of health insurance in which everyone is allowed to participate, everyone has to be included

It is how the costs are made affordable to all

As soon as you skew the pools allowing people either less apt to need services or more apt to need services, you have lost the concept of mutuality
