I believe that somewhere back in time, Congress made it illegal to use weight as a factor in setting health insurance rates, as part of a broader non-discrimination effort.

The insurance industry needs to implement a broad educational campaign to explain why some forms of insurance rate discrimination makes good sense. I certainly do not believe genetic-based discrimination makes sense, but at a minimum age and life style choices should be reflected.

As Neolib has pointed out somewhat obliquely, auto insurance rate setting reflects a wide variety of factors that result in higher costs for those who are more prone to accidents. Health insurance should be subject to the same.
One of the real weak points of the ACA is a limit on the difference in costs for old vs young. This is forcing our youth to subsidize medical costs for the aged, without any real public debate. The Reps will use this to bludgeon the law non-stop, unless the Dems can find a way to deal with this head on to hopefully get broad buy-in.
