That is not true in England, and I doubt it's true in Canada. Individual doctors may suggest that patients are too close to death, and should be sent to hospice, but there's no "rule" about it I know of. Please post such rules if they exist.

A brief perusal of the internet on the subject hasn't taken me to the "rules", but there is anecdotal evidence from England (people answering other people's questions) that you are right about there being no age restrictions in place there for receiving dialysis.

Targeting Canada doesn't bring anything up that confirms my belief and I think if my view were true it would have turned up a whole bunch of discussion on it.

So I guess you are right on this. I 'm pretty sure I read this somewhere (about restrictions) because it has stayed "locked" in my mind, but I apparently I didn't check the source and just accepted the statement as factual. So I apologize if I misled anyone here.
