State I live in has long been an Oligopoly market for health insurance... (almost a monopoly).

For private insurance the two dominant providers are --- realistically speaking --- nearly all that the local physicians and health providers will accept. They have the market sewn up. No competition to speak of.

(So of course I have one of the two big boys... not much of a choice.)
