What messes are you talking about? Like Japan? Taiwan? Germany doesn't have single payer. But many countries do. So what are the messes?

The US leads in medical innovation and most other areas because we have a robust research infrastructure, although it is and has been under attack for decades now.

Insurance companies don't do R&D.
