The breast cancer infrastructure presents almost as a church with the pink ribbon as the symbol. Lotsa entrenched interests there and a strong belief system. Like the civil rights cause, the institution can't declare victory and dismantle itself. It has to go after the last and highest hanging fruit or even conjure up figment fruit to sustain itself.

And then there is the obstacle of those who would react to any efforts at backtracking on medical services as the work of death panels. And the gender feminist sisterhood that sees a misogynist behind every pronouncement.

It might have been easier to back off back when mammograms were more painful. The new message would have provided a welcome excuse for avoiding something unpleasant. Now there's less reason to avoid the procedure.

Doctors might have better luck with the younger generations

Doctors also have a vested interest in the status quo. Some of them make money doing the tests. Others would fear alienating patients if they suggest what could be perceived as a lack of empathy or even denying expected service.

My mother died of breast cancer diagnosed at near my current age. I will probably get a mammogram or two more in my lifetime but I'm already off the yearly schedule. It's hard to break away. After all, it's "free" and you never know...
