There simply is no other way. There is always some sort of rationing, when it comes to a universally needed but limited resource. If access is not regulated by prices and competition, it becomes subject to budgetary government decisions. The latter seems to be more "fair" - since, at least in theory, everyone will have an equal chance at obtaining the services they need. The former will have the advantage of unleashing creativeness, competition, constant improvement - and ultimately, more affordable and better quality healthcare.

Make your choice... -- oh, sorry, it's been already made for you. Centralization has been the overall direction - and will continue to be so for the foreseeable future.
