Wrong questions. Who dictates what coverage the insurance companies must offer, whether or not it's needed or makes sense? Federal Government. Who mandates what companies must offer their employees, depending on how many hours they work and how many employees they have? Federal Government. Who mandates that you MUST be covered by the policies it dictates, or be fined? Federal Government.
