I was employed by a large company thru most of my 30's. I was a single woman and as I recall pregnancy/maternity services were covered, but not pediatric anything because that would have required a plan that covered existing offspring, but that was a choice that I could have made if I needed it.

In my 50's I was looking for individual health insurance on the private market. As I said, all of the policies quoted to me did not include pregnancy/ maternity coverage, or pediatric services. Why should they? I don't know how much more they would have cost had I chosen such a policy, I didn't ask at the time. I would have felt a little silly asking for a policy with pregnancy/maternity coverage, as that was so NOT going to happen.
