Technically, I doubt anyone has ever been charged $50 for a cotton swab, ever. So on that basis I don't think I'm wrong.

But on the concept, which I understood you to mean, it is fair to say that no insurance or government payer ever paid that, which is 85% of the bills that are issued. Of the remaining 15%, anyone who is willing to call and ask for it can get a discount. It is a matter of asking. So, of those in the remaining 15% who actually pay their bill! only those who don't bother to say, "don't you think 50 bucks is a little high for a cotton swab?" Might have paid it. Presumably, most people have the good sense to ask for a discount for being a self pay.
