Actually three Ohio counties (Hamilton, Stark and Cuyahoga) still make the list of the top 25 most air polluted counties (with particles measured on a year-round basis - PM2.5). Ohio air has been steadily improving though as coal plants switch to NG. Also a lot of the particulate pollution gets blown east because of the prevailing winds.

It is beyond doubt at this stage that high levels of air pollution increase asthma, COPD and cardiac morbidity and mortality. (Whether there is a difference between modest levels and low levels is still very much an open question).

But my main point is that air pollution in the US has dramatically improved over the years, and it's not had anything much to do with the free market - instead it is regulators requiring scrubbers and the like to control emissions. Absent any regulation what incentive would any utility have to use scrubbers? Scrubbers cost real money to build and operate, so it would certainly be "cheaper" for utilities to just pollute. That's what has happened in China at tremendous cost to their health and environment. (China actually tried to ban the US Embassy from tweeting actual pollution levels instead of their own made-up numbers).
