I think the PBX issue is transparent since you still have to go through your local exchange carrier anyway. I suspect, like you say, that the extra digits can be dealt with in a number of ways for a business. For consumers, custom calling card services might effectively solve the problem. Calling cards are becoming a big business now.

I also suspect that this is a temporary hassle until the network is fully configured.
