I am guessing tons were sent to hospitals and MD offices free to begin the trend. You have no idea. It is, or at least was, much more than free samples. The "gifts" showered by drug salesmen on doctors in that timeframe was, well, bribery. I am told that has been reigned in, so I don't know the current state of affairs.
