Well what you suggest is possible.. And granted the man should not be running. But many do.

But there have been some 10 incidents over the past 15 years and that would suggest there are those who decide to just play vigilante. It's possible.

But I'll go with this person has an issue that is undetected.

I will find it of interest if any pursue the possibility and if they do..well...maybe someone out there read it here to consider. Even if not it might help someone. The fMRI is a very good test. Provided you have those who have the expertise to perform etc.
