I have instructed my 3 kids never to answer any questions outside of the core school subjects they are supposed to be learning. It came in handy one day when my 14 year old was given a health questionnaire which didn't only ask how tall she was. Kid excused herself to the bathroom, called me, and asked what to do. I told her not to write anything down and politely tell the teacher to phone me if there is a problem. Of course I dont hear anything but I followed it up with an email to the principle, telling her to save paper and never give my kids a health questionnaire. The a-holes act surprised when challenged by a parent. Tough shit.
