Before your scenario's get overly complex let me interject what IBM did about 40 years ago and still has in place today..... That is, they simply announced there would be no more sick days, ....from that point on if you were sick and had to stay home you would be paid for however long you were sick (I was out for 6 weeks once after surgery), but if you are not sick do not stay at home..... The result was sick days dropped dramatically....... One of the rules was that if you were sick for 5 days or more in a row, management had the option of asking you to bring in a doctors note.....
Monday/Friday syndrome is also monitored.... The practice of paying people for unused sick days is ridiculous....and even worse is getting paid for unused accumulated sick days........
