Bill, The first single-pair 2 megs/s HDSL was released by Orckit in February 1996 (http://www.orckit.com/single.html). Before that two-pairs were needed to achieve these speeds.
The ETSI doc intro from 6/95 that Albert posted is for duplex HDSL (it says so explicitly), that is, two-pair.
Quite frankly, I didn't even know there were CAP single-pair 2megs/s HDSLs until I saw the GlobeSpan announcement. I know Performance Telecom has been selling a CAP duplex 2 megs/s HDSL (only $1,200 per line), but apart from that there are a lot of what Paradyne calls SDSLs, really single-pair HDSLs with low bandwidth (384 kb/s in each direction). PairGain is actually behind in this, as they have only the single-pair Megabit Modem to date (784 kb/s, not quite a megabit). All their other HDSLs are duplex and triplex I believe.
I valued Danny's posts more than anyone else's. He was basically accused of being biased toward CAP because he sought to introduce a bit of balance into the discussions (and because he wasn't posting enough Amati news on the TC website, even though the genuine news was all Westell/Paradyne at the time). With customers calling him about these charges, he had no option but to stop posting. He very nearly closed down the TC website. You can see the Amati archives for the whole story. I was furious at the time, as were a few others, but by then it was too late.
