Here's Ameritrades reply to my Canadian stock commission inquiry:

If the Canadian stocks are listed on an American exchange, then there
are no additional commissions. If we have to go to a Canadian exchange to buy the securities, we must go through a Canadian broker and we do pass on the added cost. The commission is $18 + .02 / share. The maximum commission is $168 on any market order and the minimum commission is $48 on any Canadian transaction. The higher cost is simply because we have to go through another broker. Thanks.
