As I pointed out, I have no idea where that graph came from.

But you can't just measure those things and index them (what appears to have been done). Because private insurance picks up the tab for underpayment of Medicare costs. Both in terms of allowable shortfalls but also with Medigap policies. Without private insurance filling these holes in the Medicare payments system, hospitals and doctors offices couldn't stay in business.

The increasing cost of private insurance (notably, since the inception of Medicare, if you notice) is a direct result of cost shifts from Medicare to private insurance. Which is what drives the cost of private insurance up since Medicare fees are price controlled.
