I have the same except I have a "rural premium" because we don't live on Oahu. Medicare plus HMSA runs $325 month instead of free and docs are leaving the outer islands as fast as they can book flights now that the ACA has gutted $700B from medicare and the Feds are making it almost impossible for stand alone specialists and docs to get reimbursed for medicare and VA treatments...unless...they saddle up with Kaiser, or some other HMO
