Only had that happen to me once, and I don't know what caused it.

This site has some good information, although it sounds like you might have done some of the things to fix it.

One thing from that site stood out to me:

There is another chance which is a delay in reading your profile files due to Antivirus scan or something else during boot up.

Based on that statement, I would update all of the installed software, then reboot in safe mode without virus protection and run a complete defrag of the hard drive.

I don't know if that will help, but maybe...
