After UE posted his comments today I went back to your original post. I don't think this is the only time the SPX is above that Keltner band by that amount. Looking at 2017 is deceptive because the chart ends here whereas the other occurrences are within a continuing price pattern where the price has continued to rise.

Look carefully at Jun 1997, Mar 1998, Jan 2004 and Feb 2011. Try to visualize the chart as though those time periods are at the end of the chart so you don't see the Keltner band continuing to rise, which obscures the extension above the band to some extent.
