I think we need a system that is culturally coherent. Systems from other cultures aren't necessarily. We can learn from other countries that do things differently whether x tends to result in y or not. We noticed, for example, that European countries typically offer mammograms every two years to our one and start later, yet their results are comparable. That tells us something we can use. But we can't tell from other systems, for example, whether or not a two tier system will suffice in our country, same for long queues, same for using lower level medical personnel for certain procedures. Nor can we tell how much the populace is willing to pay either in taxes or out of pocket.

BTW, I noticed that your boy is making it hard for foreign doctors and programs offering residencies during match season and hard for under-served areas of the country to fill openings with foreign doctors. We should be grabbing up all the doctors we can get.
