Too expensive but why not make it avail to 50+ (where things get expensive) and to pre-existing guys turned down already (with proof) at an income-based pricing grid.

First, it's not too expensive for the rest of the advanced world. We spend more than they, the proposal shifts who pays. Second, if it can be afforded for those who are expensive, extending to the young and healthy won't change much.
