Right. And they don't have to pay huge amounts for malpractice insurance, either. That can eat a big hole in income.

Remember when HMO's were the thing? Remember why they were abandoned?

They haven't been abandoned. Granted, more people are willing to pay more for a PPO, but that doesn't mean they aren't around.

You do have a point about the cohesiveness. There is a lot of division based on class. Until Obama was elected, and definitely with the rise of Trump, we can't discount racism, either.
