What possible explanation is there for this?

I've been on the bridge of two different Arleigh-Burke destroyers and have seen the set up and gotten a briefing of all the systems. There are backup systems. There are backup systems of the backup systems. There are duplications and redundancies. Even a simple GPS system that small boat owners have give off alarms when you get too close to another boat.

Why aren't they releasing the name of the OOD? Why did that person wait 14 whole minutes before sounding General Quarters? Sailors died!!!

WTF is going on?
