It is always a judgment call. Furthermore, there will always be professionals who disagree with whatever is determined.

We have a President whose access is controlled to some extent by his Chief of Staff, Kelly. Kelly spends 14 hours a day on this job. But we also know the President goes around him to contact people who deliberately seek to provoke him into rages and rants. (Bannon, Pirro, Hannity) Suppose one of these folks communicate with the President some night at 2:55 AM and fire him up about North Korea. And the President decides he'd better ACT and not wait for cooler heads to temper him. And he calls the nuclear football guy down the hall and says I want to order a nuclear strike on NK NOW!

We know how he thinks. He values ruthlessness and might think the tough guys of the world he admires and wants to be one of (Putin, Erdogan, Xi) might be impressed with him. In his mind, they might think, Gee, that Trump is a tough man of action.

Suppose the nuclear football carrier or someone else in the process makes a quick 3 AM call to Kelly, Mattis, McMaster .. whoever and says the President just ordered a nuclear attack. Suppose that person or persons quickly contacts the VP and a majority of the Cabinet and they know they realize they have to stop this NOW.

I wish this were totally unthinkable, but it's not with this guy. Frankly, I have no doubt the people I've named as well as Pence and others have had very quiet conversations about things like this happening. I wouldn't be surprised if they've already thought about the possibility of using the 25th Amendment already.

We could wake up one morning and find out Pence has assumed the President's duties under the 25th amendment.

That is not a prediction. Just a REAL possibility.
