People are gay even in places where they will kill you if you are gay.

And even if they don't kill you, in many societies you risk being ostracized, attacked and discriminated against. I can't imagine a guy waking up and thinking "You know, I really would like a dick up my ass. It might mean I lose my job, be blacklisted, attacked and beaten, but that would be worth it...".
