Its both. Both the economic problems and the social pathology help create the other.

To the extent economic problems are the source, a negative change in economic status seems to matter more than the absolute status. Plenty of people/groups/areas/economies have been poor, extremely poor by normal modern standards, without having the pathology be as extensively expressed.

Also I would say it has more to do with not having a job and not having decent prospects of getting one, than of low income.
