The CBO said the ACA would lower government deficits because the bill gamed the CBO's system to produce that result. They increased taxes at the beginning of the 10 year period the CBO would analyze and then increased spending later on in that period.

In terms of getting more people covered the ACA was overwhelmingly a matter of Medicaid expansion straight additional spending by governments and at least initially mostly the federal government since it agreed to pick up most of the cost for a time.
