I don't have an opinion about the reasonableness of the seeking alpha article but I do have questions (from the perspective on a non-biochemist). Looking at the results posted on valchlor site indicate the CAILS was not intended as a measurement for long term survival so why should it be any different for TTI-621? Also, how does CAILS take into account abscopal effects? Ointment/gel is intended for IA and IB MF patients only and results were compared over a twelve month+ period while TTI-621 were getting visible responses within days (second visit according to City of Hope clinician) and CAILS at 2 weeks. Why even compare against 12 month gel data? In addition, Sezary patients were included in TTI-621 cohorts that would even skew the comparison of Valchor/TTI-621 more. Seems to me silly to compare I/O with ointment now although they will go head to head in P3 no doubt. So those are my questions and thoughts on CAILS.

The author also thinks that it is probable in DLBCL that higher doses of TTI-621 will result in moderate improvement in efficacy with unknown SAE increase. I understand hat has always been the risk with TTI-621 or other CD-47 or any other cancer treatments. What I would like to understand is what experiences (published or otherwise) that leads to that thought when it is still in early clinical testing? Specifically, how does one extrapolate low dose results to high dose results? That is a newby question that would help me greatly.
