Thanks for answering. It's for this sort of input that I throw the question open.
I do see one eye-opening trend though: those who can pay for lotsa healthcare tend to have a higher survival rate for the "marginals' who'd not make it in an inner city or rural environment. This is a very young trend and even harder to pin down than, say one of this thread's champion whipping boys, Global Warming.
There is a darkly amusing short story based on the premise that stupid people will always outbreed the elite. I don't remember who wrote it, but it's titled "The Marching Morons". A lot of the first RoboCop movie came right out of that story.
If you like science fiction, read Greg Bear. "Queen of Angels" and "Slant". One of the premises is that medical technology becomes so powerful and so pricey (too pricey to allow central gov't to make a dent in the tab, finally killing the Great Society) that having money becomes a direct survival trait.
