The FISA warrants never directly named Trump, only people in his campaign. The judges approve only what’s presented to them. As far as they knew, Trump was not a target.

The type of warrant issued controls its scope. Some allow for surveilling the individuals with whom the target communicates, which is how Trump was surveilled.

The question is whether the judges had reason to know that Trump would end up being spied upon. If so, a hearing would have been in order. Otherwise, it probably was perceived as just another FISA warrant.

FISA warrants became so routine that I am positive that the vast majority were never questioned. It would not surprise if 90+% were rubber stamped.
